OP your problem is that the oring on the small piston is not the right one. It's supposed to be a hard white urethane oring, by the looks of it it has been replaced with a standard buna oring, and that doesn't work for these regs. Hit up crankwalk on IOG, he sells the specific oring you need.
ALSO if that doesn't work, switch the fitting to the other port. One is a gauge port and one is an input port if you switch them it will cause this issue too
they're not the best regs ever but they aren't bad by any shot.

Though it's most likely the fact that it's the wrong o-ring (or an o-ring when there should be a harder disc rather).

Also, I was mistaken on the side for the fitting and gauge, I had them reversed.
